Any chapters wanting to participate in Membership Intake, must complete the following forms:
NPHC Notice of Membership Intake
This form must be completed by September 15th (Fall) and February 15th (Spring)
NPHC Membership Intake Information
This form must be submitted Electronically two (2) week prior to beginning a membership intake process.
The form must be completed (1) week after application approval from IHQ/Regional Director. Gold Cards must be completed (1) week prior to induction stated on the Membership Intake Information.
